Grant to Help Get Healthcare to Tioga and Broome Residents
The Rural Health Network of South Central New York is getting a financial boost toward addressing health care gaps in rural and underserved communities.
The program is being awarded a three year, $160,000 Member and Community Health Improvement grant from Excellus BlueCross BlueShield.
According to a news release from the Rural Health Network, primary care patients in Broome and Tioga County serviced by Ascension Lourdes will benefit from the funding that will be used in the UR Essential program.
Staff will develop one-one-one personal care coaching and be able to refer patients to appropriate services to meet their specific needs. The effort also involves assistance with transportation, nutrition and education.
Officials say the population covered by “UR Essential” includes residents aged 18 to 64 who live in Broome and Tioga Counties, do not receive Medicaid, have identified gaps in care related to conditions like diabetes and under-use primary care services.
