Authorities say the head-on crash happened shortly after 3:30 a.m. Thursday near Exit 3 in the Town of Kirkwood.
According to the Broome County Sheriff's Office, a car traveling north in the highway's southbound lanes collided with a sport utility vehicle.
The car driver was taken to Wilson Medical Center In Johnson City, where she died.
Investigators say the five people who were in the SUV sustained minor injuries.
The southbound lanes of Interstate 81 were closed for about four hours after the crash.
Although there was dense fog in the area at the time, investigators did not know whether that was a factor in the collision.
